Natures Hand Referrals source and provide supported living accommodation throughout Gwent. At present these are through shared houses and individual flat schemes.
Supported living is a service which supports people to retain and develop their independence, whilst having the structure of support needed to live life safely within their own home. The support which is provided is unique to the individual and can be anything from a couple of hours a week to 24-hour support.
We also provide outreach support, this could be for managing health care needs within the home or accessing social opportunities within the local communities.
Our outreach services are designed to support people to remain living in their home or the family home but who need a flexible support package built around their daily lives.
Natures Hand Referrals can be made by the person wanting support, families and representatives or professionals. We will support in finding the right care package for you.
